How
to Become a Client
Please
email Dr. Ren (China) at DrWeiRen@aol.com
and express your interest in becoming a client and request a "Client
Registration Form" and a "Patient Agreement Form."
Please include your full name and telephone contact information
in the email. These forms can supplied to you via email, fax,
or mail.
You

and email, fax, or mail them back to Dr. Ren.
You may also call
his office at (212) 979-1001 to inquiry about how to become
a client as well. One will need an in-office visit ($100 USD)
or a phone consultation ($100) in order to become a client
of Dr. Ren. For phone consultations, the patient must have all
required laboratory reports, a liver biopsy, and completed patient
registration forms prior to the consultation. However, if the
patient doesn't have a liver biopsy report, Dr. Ren will still
be able to provide consultation using other lab reports. It is
best to come to New York City for an in-office visit.
If
a prospective client becomes a patient and orders medicine, the
$100 phone consultation or in-office visit fee will be waived.
